Social Security number, date of birth, home address, phone, and email. Live outside the USA? Say so on the first step and enter your international address instead — no SSN needed.

Bank routing and account numbers for the account your pay will be deposited into — or your bank's SWIFT/BIC code and IBAN if your account is outside the USA.

I-9 status details
Your citizenship or immigration status — plus your USCIS/A-Number, I-94 number, or foreign passport number if applicable. (Skipped if you live outside the USA.)

Driver's license (optional)
A photo of your driver's license speeds up your I-9 record — you can take or upload one during the form.
